Botox® for Migraine

BOTOX® (onabotulinumtoxinA) is a prescription medicine that is injected to prevent headaches in adults with chronic migraine who have 15 or more days each month with headache lasting 4 or more hours each day in people 18 years or older. The effects of the injections typically last three months.

For patients who are not receiving Primary Care Services at DB MD, the burden of getting your insurance to approve you for the procedure will be on you and your current provider. The fee for the injections is $1500. Some patients have a very small copay for this procedure, but others may be responsible for $300 or more. If you provide a letter from your insurance approving you for the injections, you are required to pay $250 at the time of injection until your insurance payment has been processed. If you cannot obtain a letter of preapproval, the entire $1500 is due at the time of service. We can provide you with the necessary information to submit to your insurance to request reimbursement, but it is always best to get the approval in writing from your insurance before injections are performed.
